Caring Hands Home Health & Rehab is proud to serve Veterans through the VA Community Care Network. We are an IDPH-licensed, ACHC-accredited agency offering skilled and non-skilled home care services throughout Illinois.
 

We partner with VA medical centers and VA case managers to provide timely, compassionate care that supports independent living and quality of life at home.

How to Get Started – For Veterans & Families

Step 1: Contact your VA doctor, nurse, or social worker.

Step 2: Ask for a referral to Caring Hands Home Health & Rehab through the VA Community Care Network.

Step 3: The VA will review and approve the referral. Once approved, our team will coordinate care with you directly.
